Requirements for Lean Six Sigma Certification
- Completion of formal Lean Six Sigma training through a recognized professional/academic training provider or a corporate (internal) training program
- Provide documentation of one or more Lean Six Sigma projects you have successfully managed as a Six Sigma Black/Green Belt or sponsored as a Lean Six Sigma Champion (see project submission template)
- 70% or more on Lean Six Sigma Certification test

Certification Fees (Canadian Currency)
Lean Six Sigma Black Belt: $375.00
Lean Six Sigma Green Belt (All): $325.00
Lean Six Sigma Champion: $275.00

Last Date to Register
Registrations and payment in full for the April 22, 2009 Certification examinations must be received on or before March 31, 2009.

Unsuccessful applications
Registrants who are not successful in passing the examination will be allowed to rewrite the certification examination on the next scheduled sitting in Fall/Winter 2009.
